Rework the wiki for the reorganized interface and new features:
- New pages: Converters, Merge & Attach, Translation, Mesh Tools,
Bone Tools, UV Tools
- Remove old pages (MMD Options, Custom Model Creation, Other Options)
- Document Fix MMD Model in Quick Access, multi-service translation,
UV/Bone tools, glTF import
- Blender 5.1 & 5.2 support (5.2 LTS recommended); drop Blender 5.0
- Point older versions to the Disroot legacy releases/wiki
- Remove Discord/website references; link the new Git repository

Banner on Home and a note in the footer so every page flags that this
wiki targets Cats 5.2.3.0 (new UI) and points older versions to the
legacy Disroot wiki.
